Make sure we have your right address before September

In August/September you will be getting an important letter about your medical records.

You must make sure that the address we have for you is your correct address or you wont get the letter.

If you have moved recently, you can check the address we have by logging in (if you are set up for online prescpription), checking the details on your prescription slip or asking at reception.
