Previous VPMC patient and not happy with the practice you were allocated?

If you are a previous VPMC patient and have been allocated to another surgery but want to register with us, please note that we can’t currently process your registration. We have to wait until the reallocation process has completed and then, we will send out new patient forms to you to complete and return.

If a large number of patients change and one practice becomes over-burdened, that practice may not be able to accept you. We are hoping few transfer and instead give their allocated practice a chance. And that those that do transfer are even between practices.
